The Greenville County Coroner’s Office in South Carolina confirmed the findings on September 22, bringing new clarity to the circumstances surrounding the Heroes and Nashville star’s sudden passing.
According to the coroner’s office, Panettiere died from the toxic effects of multiple substances, including fentanyl, 4-ANPP, alprazolam, methocarbamol and quetiapine. Her death was officially ruled an accident.
The announcement comes more than a month after the actress was found unresponsive in Greenville, South Carolina, on August 16, 2026.

The official findings indicate that a combination of substances caused Panettiere’s death.
Fentanyl, a powerful synthetic opioid, was among the substances identified in the coroner’s report. The other substances included 4-ANPP, a chemical associated with fentanyl production, alprazolam, a medication commonly known as Xanax, methocarbamol, a muscle relaxant, and quetiapine, a prescription medication.
The coroner classified the death as accidental, rather than intentional.
The findings provide the first official explanation of the actress’s death following weeks of uncertainty and speculation surrounding the circumstances.
Actress Was Found in Cardiac Arrest
Panettiere died on August 16 in Greenville, South Carolina. Emergency responders were called to an apartment after a report of an unresponsive woman.
According to the Greenville County Coroner’s Office, first responders found Panettiere in cardiac arrest. Emergency medical personnel attempted resuscitation, but their efforts were unsuccessful.
Preliminary autopsy findings released shortly after her death found no signs of trauma that contributed to her passing.
Authorities continued investigating the circumstances surrounding her death while awaiting additional toxicology and autopsy results.

Panettiere rose to fame as a child actress before becoming a familiar face in Hollywood through her roles in films and television.
